Work Permit Forms and Instructions

To obtain a work permit, please print and complete
the work permit application and take it to the District Office.

1. Print out both pages of the PDF. AdobeReader can be downloaded for free at the Adobe Website.

2. The student is to complete Section 1, the employer is to complete and sign Section 2, and the parent/guardian is to fill out and sign Section 3.

3. Have the Employer also sign and date Page 2 of the PDF.

4. Bring both pages back to the District Office; the District Office will fill out Section 4 and issue the work permit.

5. Drop off the work permit at the District Office, located at

589 W. Fremont Avenue, Sunnyvale CA 94087

The District Office will process work permits on Tuesday and Thursdays, and they will be ready for Pick-Up on Wednesdays and Fridays. If the student provides a self-addressed and stamped envelope, the work permit will be mailed as soon as it is completed.
