COACHES INFORMATION AND RESPONSIBILITIES:

1. ATHLETES MAY NOT PARTICIPATE IN PRACTICE UNTIL COACH HAS RECEIVED A COMPLETED PHYSICAL FORM!!!
· Collect all physicals prior to athlete participation in practice (physicals are valid for 1 calendar year)
¨If an athlete has participated in a previous season during the school year-note the sport on your preliminary roster
¨Check to make sure the following items on each physical form are completed:
· Both forms are signed by physician
· Parent signature
· Athlete insurance information (ATHLETES MAY PURCHASE SCHOOL INSURANCE THROUGH THE MAIN OFFICE IF THEY DO NOT HAVE THEIR OWN INSURANCE)
**You will also need to collect the CODE of Conduct form and Steroid Policy. Collect and Keep one from each student. Keep in your team binder.

2. Transfer students- pass the names of any transfers along to the Athletic Directors immediately (paperwork must be submitted to CCS to avoid game forfeiture). Also include this information on the preliminary roster.

3. Fill-out a preliminary roster form and keep track of documents (physicals/steroid form/code of conduct)
· Submit preliminary roster to the athletic directors by 10 of practice
· Submit ALL FINAL forms to the athletic director at one time!

4. Notify athletic director's of any additional coaches (volunteer or paid) that will be at your practices throughout the season


5. Submit official roster to athletic directors no later than the 10th day of practice

6. Submit official roster to Monique Orozo (attendance) Monique_orozco@fuhsd.org
* This is for release time (this is very important!) anytime you students need to miss class you need to email her!

7. Disperse official schedule to the following individuals:
· Athletic Directors
· Schedules (to Denae and Travis / Deans ) travis_hambleton@fuhsd.org denae_hambleton@fuhsd.org
·
**Notify AD's and office staff immediately of any schedule changes**

8. Schedule your team photos with VSN: 408-683-9151. Make sure to order a 16 x20 for school. (They are free to coaches and school) (Lori will schedule Winter and Spring)

9. For information about your sports by-laws and general information:
· About the league go to www.scval.org
· About CCS go to www.cifccs.org

10. Attend the yearly coaches BBQ/meeting held the second week of school (September 5)

11. When ordering uniforms/gear be sure to adhere to the official school colors: Purple, Gold, White.


12. Apply for CCS sanctioning if hosting your own tournament at MVHS (www.cifccs.org)

13. (After the fall BBQ 7:30 p.m.) Organize a parent meeting to disperse schedules, get help with carpooling to events (if necessary), and make announcements regarding athletic boosters involvement & athletic support donations ($60)

14. Notify Athletic Directors when an athlete quits the team or if serious injury occurs

If you have equipment needs make sure to convey them to the Athletic Directors
15. Take charge of fundraising money for your own program to help fulfill some of the minor purchases that your program requires

16. Submit all reimbursement requests to Judy Ma judy_ma@fuhsd.org as soon as possible.

17. For use of facilities Lori Will set the master calendar for practices in gym usage. If your practice times change always update Lori so the online calendar can be updated.

18. Be sure to get ASB approval of all fundraisers, submit appropriate paperwork to Denae Moore in the ASB office

19. Submit all results to the San Jose Mercury News (920-5354)

20. Notify Lori Graham of any outstanding athletic accomplishment or important upcoming events:

21. Be Aware Grade due dates and when to submit Award needs:.
· Athletic Points: End of season
· Grades: have six grading times in the entire year. PLEASE personally grade each student when it is in your season. 'All As doesn't work. We must make sure each student that you give a grade for suits up, practices with the team, and is competitive on some level.
· Awards: During the last week of season play. NO Later then last game
· Athlete of the year nominations

25. Eligibility Information:
· Athlete's must pass 5 classes and maintain a 2.0 GPA to stay eligible for competition
§ Athletic Directors will determine the eligibility of athletes on your roster at the beginning of each season, and whenever new grade reports are released
· An athlete is put on probation when passing 5 classes but not maintaining a 2.0 GPA, or when they have a 2.0 GPA but are not passing 5 classes
§ Athletes cannot be on probation for 2 consecutive grading periods

26. All coaches must attend the sports awards night in May for the MVHS athletic presentations. All Senior coaches are invited to attend the Senior Awards Night

27. Have a team banquet at the end of the year. Disperse all individual awards at the team banquet
The school provides each team with 2 awards (Coaches & MVP) any other awards you wish to hand-out will come out of your own sports financial account

BLOCKS to hand out must be requested by YOU. Please hand them out at your banquet
